As an older adult, dealing with memory, mood and movement concerns can be difficult. Aging while remaining safe and independent can be a challenging process. Community's Healthy Aging Transition Services, or HATS, clinic can help you and your loved ones create a plan of care to successfully navigate the aging process. What Is the HATS Clinic?
The HATS geriatric consult clinic is an extra layer of support for older patients. The clinic doesn't replace primary care, but instead works alongside you or your loved one's primary care provider by sharing lab work, medical history and recommendations. This partnership helps both the HATS clinical experts and the primary care provider deliver more personalized care for an individual's specific aging concerns and lifestyle.
The HATS clinic is also designed to give agency to older patients. When dealing with the aging process, older patients can often be left out of decisions about their health. At Community, we want to make sure that our patients are respected, safe and independent at any age. By having conversations about the aging process earlier, patients can control their future before memory loss, dementia or illness prevents them from doing so.
Who Benefits from the HATS Clinic?
Generally, the HATS clinic serves patients aged 65 and older. The biggest population of patients have memory concerns. But the clinic isn't just "one size fits all". HATS also addresses mood changes that can be related to dementia or other underlying health conditions, helps patients manage medications, assists with mobility needs, and more.
Patients experiencing any of these issues or who have other concerns about aging can be referred to the HATS clinic in several ways. A primary care provider might refer a patient, but patients can also be referred by friends and family, or even make a self-referral when concerns arise.
What Is a HATS Appointment Like?
After being referred to the HATS clinic, patients engage in a pre-visit phone call. Family or caregivers that can help shed light on the concern may be included.
During the first appointment, patients undergo a comprehensive geriatric assessment to help determine a path forward. The HATS team speaks with the patient and caregivers about short- and long-term goals of care, discusses medical history, and works towards an appropriate diagnosis. It's important to remember that the aging process isn't one size fits all and care will be adapted based on the patient's unique concerns.
The patient and caregivers are then invited back for a second visit to review results of the initial assessment, discuss any additional screenings or tests, and implement a plan of action. And, in coordination with a primary care provider, follow-up appointments are arranged as needed.
