This advanced practice provider will be focused on the well-being of Onsite employees and their beneficiaries.

Community Health Network has an excellent opportunity for an experienced, family medicine certified advanced practice provider to practice PRN at our Employer Health Onsite clinics. Minimum two years of experience as an advanced practice provider is required.

Job Duties

• Routine check-ups and preventive care

• Diagnosis and treatment of illness or injury

• Managing chronic or complex health conditions

• Medication management

Education

Educational background consistent with requirements for licensure and certification with the State of Indiana.
