Catalyst Award for Practice Innovation | Community Health Network

The Catalyst Award for Practice Innovation is presented to Community Health Network physicians and APPs who exemplify leadership in adopting and implementing new or improved workflows that enhance the provider experience. These individuals are early adopters and change agents bridging ideas into action and helping their teams thrive.

We want to recognize these individuals or teams not only for their commitment to improvement, but for the inspiration they provide to others through their willingness to learn, adapt, and lead change.

Any Community Health Network employee may nominate a physician or APP who has demonstrated excellence in the adoption of practice-enhancing workflows through any of the following:

Leading or supporting the successful implementation of improved clinical or operational workflows.
Serving as a resource or role model for peers during transitions or process changes.
Contributing to a culture of continuous improvement through shared learning and collaboration.
Supporting the well-being of colleagues by helping make every day work more sustainable, efficient, or satisfying.
