Advanced Practice Provider Students - Clinical Rotation Request

We offer high-quality clinical education experiences for Advanced Practice Provider (NP, PA, CNM, SRNA) students from affiliated schools who meet specific criteria. Practice settings for qualified APP students to complete their clinical education include a variety of settings in primary and specialty care. Requirements, application deadlines and online application link may be found below.

How To Request APP Clinical Rotations

Thank you for your interest in Community Health Network as a site for a portion of your clinical education. We are happy to evaluate your request for placement. Our hope is that APP students who come to Community have an exceptional educational experience. In order for us to provide that experience, please review this information carefully and follow the steps below.

All APP students who are seeking placement for a clinical rotation at Community must complete our online clinical placement application. See application details and deadlines below before submitting a request.

If you have already completed the application and need to notify us of a placement or make a change to your original request form (i.e., name, graduation date, etc.), please contact NPPAStudents@eCommunity.com.

Who Can Apply?

APP student degrees accepted for placement include:

  • Nurse practitioner (NP)
  • Physician assistant (PA)
  • Certified nurse midwife (CNM)
  • Student registered nurse anesthetist (SRNA)

Application Requirements and Timelines

For Students Seeking Preceptor Assistance

  • NP: All NP students must submit their preceptor assistance application for the upcoming rotation(s) prior to the deadline(s) below (see chart). Late applications will not be accepted. Student notification will take place on or before the date listed.
  • CNM: Please reach out to NPPAStudents@eCommunity.com regarding CNM placements. Spots are limited and there is a waitlist for students.
  • PA and SRNA: Please reach out to your PA or RN Anesthesia school coordinator for next steps. We do not accept applications for PA or SRNA placement assistance, as we work directly with the schools to offer placements.

For Students Who Have a Preceptor

  • NP, CNM, PA: All NP students with an assigned preceptor must submit their full application no less than one month prior to the start of their first clinical rotation.
  • SRNA: Preceptors are assigned based on the schedule set in agreement between your school and Community. All SRNAs scheduled for Community must submit their full application no less than three weeks prior to the start of their clinical rotation.

APP Clinical Placement Frequently Asked Questions

Q: If I am not a current Community Health Network caregiver, can I apply for placement assistance?
A: Yes. While Community Health Network prioritizes our employed caregivers for placements, we do accept applications from external students.

Q: Can I find my own preceptor at Community Health Network?
A: Yes. If you have already established a relationship with a Community provider who has agreed to preceptor you, please reach out NPPAStudents@eCommunity.com for next steps. We do not encourage “cold calling” practices and only permit you to reach out to providers with whom you or your school have an established relationship.

Q: When do I need to submit my application?
A: If you have a preceptor, you must complete your full application no later than 30 days prior to the start of the rotation. We abide by our application deadlines and cannot make exceptions. Please review the chart above regarding deadlines.

Q: Does Community Health Network work with my school?
A: We only accept students from universities with an active, fully executed legal affiliation agreement. You can check our current list of partner school above.  If your school is not listed, we may not be able to accommodate your request at this time.

Q: Do you establish new affiliation agreements?
A: New agreements are considered on a case-by-case basis and must align with our strategic recruitment goals. Please note that the legal review process for a new agreement can take several months and is not guaranteed. 

Q: Does submitting a request guarantee a placement?
A: No. Due to high demand, submitting an application does not guarantee placement. We work hard to match as many students as possible based on preceptor availability and capacity.
