Make a Difference

The Care Coordinator Pathway student will learn to engage with individuals and families to improve their health and wellness, live self[1]directive lives, and strive to reach their full potential. The LSC monitors and provides care coordination and life skills training and development, in collaboration with the care team, to youth and/or adults with co-occurring mental health and/or substance use disorders[1]including those with serious and persistent mental illness.

EDUCATION · High School Diploma or GED

Must be enrolled in a bachelor level program in partnership with the Care Coordinator Pathway. (Required)

 · 4 year / Bachelor's Degree Bachelor's Degree required for anyone hired May 1, 2023 and after. (Required)

 LICENSE & CERTIFICATION REQUIREMENTS

· Required Certifications/Licensures: (Current Indiana Driver's License with valid vehicle insurance)

 

EXPERIENCE

 · 0+ years: Enrollment in a bachelor level program in partnership with the Care Coordinator Pathway. (Required)

ESSENTIAL FUNCTIONS (JOB DUTIES)

 · Patients First: Collaborate as part of a multidisciplinary care team by supporting the comprehensive psycho-social assessments. Ability to help clients regulate emotions. Document services provided in Electronic Medical Record (EMR) based on policies and procedures. Page 2 of 4

· Relationships: Collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving.

· Integrity: Comply with all applicable Behavioral Health Care Service policies and procedures.

· Inclusion: Ability to work with clients in various stages of change who need assistance regulating thoughts and emotions.

· Diversity: The idea candidate will be able to acknowledge, value and celebrate collective experiences and differences of patients, other caregivers and the communities we serve.

· Excellence: Actively involved in professional development and education, including mandatory training.

· Department accountability: Attend at least 80% of the Clinical Group Supervisions offered by Care Coordinator Pathway Program. Attends on-site supervision with field instructor as required by school. Ability to use supervision appropriately. Ability to document services. Meets coursework expectations as required by their bachelors program. Completes a minimum of two shadowing opportunities in substance use disorder programs, if not already placed at a substance use disorder treatment site.

· Adhere to Community Fairbanks Behavioral Health and/or program partner policies, procedures, values, and mission, including maintaining professional standards in attitude, appearance, and engagement during all virtual and in-person activities related to program and practicum experiences.

· Individuals may not perform all of these duties, or may perform additional, related duties not listed here.
