Expert Care for High-Risk Pregnancies

When you’re diagnosed with a high-risk pregnancy, you’ve got an additional layer of support at Community Health Network. Our Maternal-Fetal Medicine Care center is a high-risk obstetrical clinic where you’ll get comprehensive care for the risk factors relevant to your pregnancy and delivery. Perinatal Nurse Navigation Program

This program is designed to support women throughout their pregnancy and for up to a year after the baby is born.

We help by connecting mothers with:

  • Community resources (like help with food or housing).
  • Special doctors and other experts.

This helps us follow the best care practices to ensure births are as safe and healthy as possible.

How the Program Works

  • Our special nurses, called perinatal nurse navigators, run the program. They work across all our hospitals and clinics.
  • They give personal and organized care to women who have a higher chance of:
    • Giving birth too early (preterm delivery).
    • Having problems after birth (postpartum complications).
  • This higher risk can be due to life challenges, money worries, or other medical problems like diabetes or high blood pressure (hypertension).

We want to meet each woman’s unique needs. We can do this through video or phone calls (telehealth) or by meeting you at the doctor's office.

Our main goal is simple: to help you have a healthy pregnancy and a healthy baby.

Who Can Participate?

To join the program, you must be a patient who is getting prenatal care (care before the baby is born) at one of our OB/GYN clinics and be referred directly into the program by someone on your care team.

Referral Process

  • A prenatal care nurse will check your medical and social needs before or during your first visit and will refer you to the program if they determine extra support is needed.
  • Any person on your care team can also refer you to the program at any time during your pregnancy.

Care Provided

The perinatal nurse team helps you with:

  • Breastfeeding support (getting help with feeding your baby).
  • Safe sleep education (learning the best ways for your baby to sleep safely).
  • Getting organized care with other specialists if you need them.
  • Working with community groups to help mothers and newborns with their needs.
  • Advice on making a healthy home for your baby.

Our team is always reviewing the program to make sure we are helping pregnant women in the best way possible. If you have any questions, please contact your doctor or nurse.

Virtual Perinatal Support Groups

Having a baby is a joyous occasion, but it doesn’t always feel like it. You’ve probably heard the term “postpartum depression” — more accurately called perinatal depression and anxiety — but you might not realize how common it is. At least one in seven new moms will experience the condition, and 60% of the time it starts before or during pregnancy.

A great way to get support is to talk to people just like you. Our Virtual Perinatal Support Groups meet once a week for about 45 minutes, and offer you a chance to meet other new moms who are also struggling with mood changes or the transition to parenthood.

Sessions are free to attend, even if you aren’t a Community patient. You don’t even have to worry about registering — just email us at bhintegration@eCommunity.com for details on how to join:

New and expectant moms can make Community's own FigLeaf Boutique their one-stop shop for all their nursing and infants needs as well as being professionally fit for nursing bras. If you are a patient at Community Hospital East, North or South, a fitter from FigLeaf can fit you in your maternity suite. This is a complimentary service. FigLeaf also offers a selection of specialty gifts and oncology care items.
