Meet the HATS Team

Braca Cantor, MD

Braca Cantor, MD, geriatricianBraca Cantor, MD, is a proud Hoosier. She received her undergraduate education from DePauw University in Greencastle, IN and it was there she was inducted into the prestigious honor society, Phi Beta Kappa. She then obtained her medical degree from Indiana University School of Medicine in Indianapolis. Her internal medicine residency and a subsequent geriatrics fellowship were completed through St. Vincent in Indianapolis.

Dr. Cantor is a member of the American Geriatrics Society, Indiana Geriatrics Society, American Medical Association, and American College of Physicians. In 2018, her accomplishments in the field of geriatrics were honored when she was named one of the "100 Hoosier Heroes" by the United Way of Central Indiana.

Dr. Cantor brings a unique passion to the field of geriatric care. She has a special interest in cognitive disorders and mental health, with a priority to improve the quality of life for all her patients.

In her free time, Dr. Cantor loves spending time with her husband, two children and huskies. She enjoys reading, listening to oldies and trying new local restaurants.

Molly Campbell, DNP, RN, AGNP-BC
Nurse Practitioner

Molly Campbell, RN, NP, Touchpoint HATS clinicMargaret "Molly" Campbell is the executive director of population health and transformation services at Community Health Network. Molly brings more than 15 years of healthcare experience to this role. She works closely with executive leadership, medical staff, employees and the public to support Community Health Network’s mission, vision and values in promoting the development of a leading geriatric and palliative care model.

Molly received her bachelor of arts degree in gerontology from Miami University in Oxford, OH. She then entered a three-year program at Yale School of Nursing in New Haven, CT, to obtain her RN and master’s degree in nursing. In 2022, Molly completed her doctorate of nursing practice degree from Purdue University. Molly’s nursing career has always been focused on the care and well-being of older adults, spanning a variety of healthcare settings including inpatient and outpatient care. She is proud to support and lead the Palliative Care team, specialty geriatric programs, and home-based division at Community Health Network.

Molly’s professional certification and associations include American Nurses Credentialing Center (ANCC) Certification in Adult and Geriatrics; American Geriatrics Society member; Indiana Geriatrics Society member; and American Academy of Nurse Practitioners member. She also serves on the board for the Indiana Geriatrics Society.

Molly and her husband, Noll, are the proud parents of Beckett and Dorothy. In her free time, she enjoys spending time with family, staying physically active, traveling, reading, and going to sporting events and concerts.

Dedra Goff, MSN, APRN, FNP-BC
Nurse Practitioner

Dedra Goff, NP with Touchpoint HATSDedra Goff is a nurse practitioner with the Health Aging Transition Services (HATS) clinic in Noblesville, IN. Dedra brings several years of nursing expertise by focusing on support for patients and families related to the aging population. She received her bachelor of science in nursing at Harding University where she graduated Magna Cum Laude. After spending 26 years as a registered nurse in the acute care setting and outpatient setting, she received her master of science as a family nurse practitioner from West Virginia University School of Nursing.

Prior to joining Community Health Network, Dedra provided care as a nurse practitioner in an outpatient primary care setting, then in skilled nursing facilities. While supporting patients in skilled nursing facilities she focused her care on patients with dementia and other neurocognitive disorders. Dedra joined the HATS team to further support patients and families to improve the overall health, functionality, and quality of life in our geriatric communities during this stage of life.

Dedra is a member of American Association of Nurse Practitioners, AANP, as well as the American Geriatrics Society, AGS.

In her free time Dedra enjoys cooking, traveling and spending time with her family.

Twyne’a Manning, DNP, AGPCNP-BC
Nurse Practitioner

Twyne'a Manning, DNP for HATSTwyne’a is a nurse practitioner for the Healthy Aging Transition Services Clinic located in south Indianapolis. She has several years of nursing experience since earning her bachelor of science in nursing from the University of Louisville in 2012. She spent time working in the acute care setting as a floor nurse, and then as a travel nurse. Afterwards, she earned a doctoral degree at the University of Illinois at Chicago to become a doctor of nursing practice (DNP) focusing on adult and geriatric medicine. Before joining the team, she spent two years in home-based primary care.

Twyne’a is a member of Gerontological Advanced Practices Nurses Association (GAPNA). She is excited to be able to focus more on geriatric healthcare. Her passion for geriatrics stems from wanting to allow this population to have more of a voice and involvement in their healthcare.

In her free time, she enjoys being with family and friends, traveling, and attending sporting events. Twyne’a often goes by the nickname, Ne’a.

Rachel James, MSW
Clinical Social Worker

Rachel James, social workerRachel is an Ohio native who received her undergraduate degree in gerontology from Bowling Green State University in 2009. While in undergrad, a friend convinced her to take an Intro to Gerontology course and from that she found her passion. Rachel moved to Indianapolis in 2013 where she worked as a social worker for eight years in long-term care for companies such as American Senior Communities and Communicare. Long-term care is where she gained the majority of her social work experience and pursued her passion for working with older adults. Rachel has also worked as an STNA/CNA in both Ohio and Indiana as well in activities as a director.

She obtained her master of social work degree from Indiana Wesleyan University in 2021. During her master's program she gained more experience with her practicums in home health, hospice and palliative care. After graduating, she joined the HATS team. Rachel enjoys helping families navigate the world of geriatrics. She believes that compassion and understanding go a long way.

In her spare time Rachel enjoys traveling, spending time with family and friends, and doing yoga.

Todd Wagoner, MSW, LCSW, C-ASWCM
Clinical Social Worker

Todd Waggoner, MSW, LSWTodd is a master’s level clinical social worker with over 25 years of healthcare experience in hospital discharge planning and, most recently, serving as geriatric social worker for the Healthy Aging Transition Services. Todd's primary responsibilities are to facilitate neurocognitive psychological testing for individuals with cognitive impairments, provide clinical counseling to address depression and anxiety in older adults, and address caregiver needs through individual and group counseling.

Todd also serves as adjunct professor for the Department of Social Work at Taylor University (Upland, IN). He speaks regularly to both formal and informal groups on subjects such as dementia, caregiver stress and aging as it relates to mood disorders, while serving on Taylor University’s Department of Social Work Community Advisory Board.

On a personal note, Todd has a voracious appetite for reading and is honored to be given this mission of serving individuals and families who are met by the challenge of experiencing both cognitive and mood disorders during the aging process.

Brittney Waters, BSN, RN
Registered Nurse

Brittney obtained her associate of science in nursing in 2013. During this time, she worked as a bedside nurse in a long-term care facility. Brittney then went on to obtain her bachelor of science in nursing in 2019 from Indiana Wesleyan University. After working as a bedside nurse in long-term care, Brittney joined the HATS Team in May of 2019. She found her passion for geriatrics while in nursing school. This passion only grew as she worked with the geriatric population during her time as a bedside nurse.

Brittney was inducted into the Sigma Theta Tau International Honor Society of Nursing, Eta Chi Chapter, in April of 2019. Brittney completed the NICHE, Nurses Improving Care for Healthsystem Elders, geriatric resource nurse training in November of 2019.

In her spare time, Brittney enjoys spending time with her family and friends. She also enjoys traveling, especially to her favorite vacation destination, Panama City Beach, FL.

Marcene Williams, RN
Registered Nurse

Marcene received her nursing degree from Ivy Tech Community College and spent the last 10 years working in a long-term care setting. In her journey, she has developed empathy in caring for older adults. She has become an advocate for those in their golden years, with a focus on increasing their quality of life and safety, and helping others understand them. She can connect with caregivers as they assist their loved ones through the aging process.

Marcene joined Community Health Network in 2021, and is pleased to be part of the Healthy Aging Transition Services south team.

Marcene enjoys time with her husband, Ken, and their children. She especially enjoys being outdoors, gardening, hiking, and reading.

Sydney Wagoner, MSW
Care Coordinator

Sydney Wagoner, MSWSydney obtained her bachelor’s degree in social work from Taylor University and recently graduated from the University of Houston with a master’s in social work with a concentration in gerontology. Sydney was inducted into the Phi Alpha Honor Society in April of 2020. She began working in geriatrics as a care manager for CICOA Aging & In-Home Solutions before joining the HATS team.

Sydney is a member of the National Association of Social Work (NASW) and is passionate about working with older adults. She wants to ensure that all patients are given the best quality of treatment along with assisting caregivers by providing support during their time with HATS.

Sydney lives in Indianapolis with her sister and has been an avid tennis player since the age of ten.
