Immigration Physicals

This page explains how to prepare for an immigration physical and what to expect at an immigration physical appointment.

Dr. Maurice Henein is a civil surgeon who can perform immigration physicals. We welcome all individuals who would like to have their exam done by Dr. Henein.

Prepare for Appointment

Appointment Type and Length

  • Appointments are scheduled for 30 minutes.
  • Starting January 1, 2025, Dr. Henein will only be completing immigration physicals on Monday afternoons, Wednesday mornings or Wednesday afternoons. This will allow him to continue to provide primary care appointments for his patients.
  • Most people need the full immigration physical and exam.
  • However, refugee or derivative asylee adjustment of status applicants under Immigration and Nationality Act (INA) section 209 and K non-immigrant visa holders applying for adjustment of status will only need an Immunization Update/Refugee Physical.
  • We will try and schedule families together.

Cost

  • Immigration Physical - $250 due at the time of service.
    • This service cannot be billed to insurance.
  • Immunization Update/Refugee Physical - $70
    • For those who have legal status as a refugee and need only an immunization update. This is a different service than the full immigration physical.

Language/Interpreters

Immigration physicals are conducted in English.

  • Patients that do not speak English MUST bring an interpreter to the appointment. The I-693 form must be signed by an interpreter and this cannot be done with a phone/iPad interpreter.

What To Bring

  1. USCIS Form I-693
    1. Fill out Parts 1 and 2. Do NOT sign on page 2 until at the appointment.
    2. Complete Part 4 if someone helped you with Parts 1 and 2.
  2. Vaccine Records - Bring any and all vaccine records that you have to the appointment. We will be able to access the Indiana state registry for any vaccines given in Indiana at the time of the visit to assist.

At the Appointment

At the appointment, we will determine which lab tests and vaccines are needed.

Lab Tests

  • The exam requires testing for tuberculosis (TB) and syphilis for most adults and that is done with blood work.
    • If you have been treated for TB in the past, we will need those records and you will then need to have a chest x-ray completed. This can be completed prior to your visit or can be ordered on the day of your visit.
    • If your bloodwork for TB is positive, you will then need a chest x-ray prior to completion of the exam paperwork.
  • Those between ages 18-24 also need a urine test for gonorrhea.
  • These tests will be ordered the day of your visit and should result within 3-5 days at the lab.
    • If you have insurance, these tests are typically covered.
    • If you do not have insurance, the office can recommend some lower cost lab services at the time of your visit. The lowest cost for uninsured individuals for the immigration physical, labs and vaccines are at the Marion County Health Department.

Required Vaccines

Vaccines may be necessary to complete the form and we will determine that at the visit. Those will be billed through insurance if you have insurance. Costs for vaccines are generally lower at the Marion County Health Department.
