Our Community Hospice physicians and nurse practitioners are dedicated to patients in the hospice journey. They help develop a plan of care with the patient, family, and the patient’s physician to best meet the patient’s wishes.
Tom Ledyard, MD
Tom was raised in a large family in northwest Indiana whose values emphasized cheering for the underdog and helping the underserved. He received his bachelor degree at Indiana University and medical degree from Michigan State University. He completed his residency in family medicine at Community Hospitals and an ethics fellowship at IU. He met his wife, also a family doctor, in college at summer camp for handicapped children.
The strong influences of family, education and mentors has lead them to the path of working with the underserved in Indianapolis, Kenya, and in multiple community health centers. He tries to pass forward his passion for care of marginalized populations through medical education. His practice currently focuses on end of life care - helping patients and families find peace in transition.
He has four children: a writer, boat builder, OB/GYN and entomologist. He enjoys being outside, working with wood and serving on non-profit boards.
Abi Duncan, FNP-C
Abi graduated from Anderson University with her bachelor of science in nursing in 2014. She worked as a nurse in the emergency department and hospice prior to graduating with her master's degree. Abi attended Indiana Wesleyan University and graduated with a master of science in nursing in the family nurse practitioner program in 2020.
Abi is ecstatic to continue her work in the palliative care and hospice setting with Community Health Network. Her passion is to help support patients and families living with serious illness achieve their goals and focus on comfort to allow for quality days.
Devin Hartzler, MSN, FNP-C, ACHPN
Devin is board certified as a family nurse practitioner with additional hospice and palliative care certification as an Advanced Certified Hospice and Palliative Nurse. She has had sixteen years of nursing experience, starting her career as a diploma-trained licensed practical nurse. She has since moved her way up the nursing career ladder obtaining her associate’s degree in nursing, bachelor’s degree in nursing from University of Indianapolis, and her master’s degree in nursing, also from University of Indianapolis. She has worked in a variety of settings including long-term care, acute care, primary care and hospice
Realizing the importance of quality of life, as well as a passion for caring for end-of-life patients, she has dedicated the past 10 years of her career to the hospice realm, initially as a hospice registered nurse and now as a hospice nurse practitioner. She is and End-of-Life Nursing Education Consortium trainer through the American Association of Colleges of Nursing.
In her free time, Devin enjoys gardening, rescuing animals and spending time with her son.
