COPD Care Clinic

Treatment of chronic obstructive pulmonary disease at the COPD Care Clinic is designed to focus specifically on your COPD diagnosis, and to do everything possible to improve your breathing and your overall health. We offer a unique, multidisciplinary approach to make sure your breathing is the best it can be. We have a standardized assessment that we use to tailor your individual COPD therapy plan to reduce the risk of exacerbations and keep your symptoms controlled so you can enjoy your life.

Your Visit

At your first clinic visit, you will meet with our pharmacist, respiratory therapist and one of our providers. Together, we will:

  • Have you perform spirometry. This breathing test will help us stage your COPD severity.
  • Review your medical history.
  • Talk with you about what COPD is and how it can be treated.
  • Educate on inhaler usage.
  • Review the COPD plan checklist and provide you with a personalized COPD Zone plan to take home with you.

COPD Plan Checklist

It may take more than one visit to cover everything on the checklist and not every patient needs everything on this list.

  • Medications: Which medications are right for you? What barriers do you have to picking them up?
  • Exacerbation prevention: What can we do to prevent a flare up?
  • Sick plan: What is your “Action Plan” if you get sick?
  • Sleep apnea: Are you high risk and do you need a sleep study?
  • Smoking cessation: Have you stopped smoking? If not, how can we help?
  • Pulmonary rehabilitation: Would you benefit from pulmonary rehabilitation and be willing to participate?
  • Oxygen: Do you need and qualify for oxygen?
  • Immunizations: Are you up to date with your immunizations?
  • Advanced directives/POST Form: Have you completed advanced directives or a living will?
  • Palliative care: Could palliative care be helpful to optimize your quality of life?

COPD Care Team

Medical Director/Provider

Kaitlin received a bachelor of science degree from Ball State University, a master's degree in medical sciences from Lincoln Memorial-Debusk College of Osteopathic Medicine, and a master of business administration degree from University of Southern Indiana. She has worked inpatient for the Community Pulmonary Critical Care Medicine group at Community Hospitals East, North and South since 2014. She served as the advanced practice provider supervisor for Community South for two years before taking on the medical director role for the COPD Care Clinics.

Kaitlin has designed the COPD Care Clinic to use a comprehensive, evidence-based approach to help patients meet their goals and increase their quality of life. She is passionate about increasing access of care for patients, and helping them navigate barriers in the healthcare system. Along with the multidisciplinary team, she is committed to improving the lives of the patients by alleviating their symptoms and improving the function of their everyday lives.

Sara Bohan, PA-C, COPD Clinic providerSara Bohan, PA-C
Provider

Sara received a bachelor of science degree in biology at Indiana University and her master's degree in physician assistant studies from Butler University. She has worked inpatient for the Community Pulmonary Critical Care Medicine group at Community Hospitals North and East since 2015 and served several years as advanced practice provider supervisor. In addition to her clinical roles, Sara also works with Community's Informatics Department as a provider informaticist.

As a provider in the COPD Care Clinic, Sara works one-on-one with patients to identify their barriers to healthcare, provide in-depth patient education and create tailored COPD treatment plans. These treatment plans are practical, affordable and medically optimized based on the most up-to-date GOLD guidelines. The goal is to enhance patient compliance and maximize symptom control, thereby reducing the frequency of ED visits and hospitalizations. She also collaborates with other members of the patient’s healthcare team, as well as with network programs.

Mark Frazee, PAMark Frazee, PA-C
Physician Assistant

Mark received his bachelor of science degree from the University of Southern Indiana, a master's degree in physician assistant studies from Butler University and a master's degree in business administration from the University of Southern Indiana. He has worked with Community Pulmonary Critical Care Medicine group since 2009. Additionally, he has held several leadership roles at Community and currently serves as executive director for medical specialty integration.

Mark joins the COPD clinic passionate about caring for patients from a wholistic perspective. While practicing evidence-based medicine, he understands the importance of still meeting individual needs and offering the necessary support for patients and their families to navigate the complexities of the modern healthcare system. He believes in the multidisciplinary, team-based approach the COPD clinic utilizes and looks forward to further serving the COPD community. 

Sarah Kain, PharmDSarah Kain, PharmD, BCPS
Pharmacist

Sarah received her doctor of pharmacy and minor in gender, women, and sexuality studies degree from Butler University College of Pharmacy and Health Sciences in 2017. She completed her PGY1 pharmacy residency at Franciscan Health Indianapolis and PGY2 ambulatory care pharmacy residency at St. Louis College of Pharmacy & SSM Health St. Mary’s Hospital. She is a board-certified ambulatory care pharmacy specialist and tobacco treatment specialist. She joined Community Health Network in July 2019 as a clinical pharmacy specialist in ambulatory care with her current practices in primary care, pediatrics and COPD care clinics. She currently serves on multiple committees and workgroups within Community Health Network and professional pharmacy organizations; she also precepts and mentors pharmacy residents and students year-round.

She serves the COPD clinics on two half-day sessions each week to collaborate with the team to optimize the COPD medication regimen, identify opportunities for necessary immunizations and counsel on nicotine cessation. Sarah also provides patient education and ensures patients can afford and access their COPD medications. She collaborates with clinical pharmacists across Community to address additional medication therapy opportunities to improve overall patient outcomes. Sarah is committed to improving the lives of people within our community and help them truly live more.

Shannon Ladislas, respiratory therapist, COPD ClinicShannon Ladislas, RRT
Respiratory Therapist

Shannon is the clinic respiratory therapist and has been a practicing RRT for more than 15 years. She received a bachelor of science degree in marketing from Purdue University and a bachelor of science degree in respiratory therapy from Indiana University.

She provides quality care to patients by conducting spirometry, titrating oxygen, counseling on inhaler usage and tobacco cessation, administering breathing treatments, educating on breathing exercises, and helping to determine treatment plans in consultation with the pharmacist and providers.
