Call 317-621-6550 to schedule an appointment. State you are a member of the Community Cardiovascular Specialty Care program. The concierge will connect you to an approved Community Heart and Vascular care office. See available locations.
Consult with the Community Heart and Vascular doctor to plan your care. See approved list of services and programs.
Schedule additional services at approved Community Heart and Vascular care locations. The cardiology office(s) will provide education and preparation for any testing, procedures and/or surgery.
Complete any recommended tests, imaging or services at an approved Community Heart and Vascular care location; schedule procedures or surgeries at Community Heart and Vascular Hospital.
