Schedule Your Mammogram

Annual screening mammograms are the most effective tool for early breast cancer detection and peace of mind. At Community Health Network, we make it convenient to get this essential screening by offering walk-in appointments and extended hours at locations near you throughout Central Indiana. You can schedule your mammogram online below to choose a time that works best for you.

If you prefer personal assistance or have questions, our team is available to help with scheduling at 317-355-4680.

Schedule a Mammogram Online

Answer a few questions and then select a location and time for your mammogram appointment. Press "Start Over" to go back to the first question of the questionnaire. After you select which breast(s) should be screened, you will be able to select your preferred date and location.

The final step is to fill out your personal information and click on Schedule Appointment. You will receive a text message confirming your appointment. If you are not sure if your appointment has been scheduled, please call the location to confirm before arriving.

Are you age 35 or older?

If you are under age 35, please do not schedule a mammogram online. If you are under age 35 and have a provider order for a mammogram, or if you have questions about your eligibility for a screening mammogram, please call to schedule. Thank you!
