 | Sunflower AwardThe Sunflower Award honors other clinical caregivers who provide outstanding direct patient care and service to patients and their families. Anyone can nominate. |
 | Lily AwardThe Lily Award honors patient service representatives who provide outstanding appointment scheduling service to patients and their families. Anyone can nominate. |
 | PHIL AwardThe PHIL Award honors outstanding respiratory therapists who provide exemplary care and treatment for patients with respiratory illnesses. Anyone can nominate. |
 | Thank Your Doctor, Physician Assistant or Nurse PractitionerAt Community Health Network, our physicians, physician assistants and nurse practitioners give their best every day to care for you and your family. If one of them made a difference in your visit, take a moment to tell them. Your thanks will be sent to them directly via email, so they know the impact they had on your care. |
 | PRIIDE AwardThe PRIIDE Award recognizes staff/caregivers who demonstrate our core values: Patients First, Relationships, Integrity, Inclusion, Diversity, and Excellence. All staff/caregivers are eligible for nomination for the PRIIDE Award. However, the PRIIDE Award is primarily designed for staff/caregivers who are not already included in or eligible for the specific award categories outlined above. |
