EAP services are available to you. To schedule an EAP appointment, please call 317-621-7742 or 800-543-4158. To learn more about EAP, please contact your Human Resources Department or visit eCommunity.com/eap.
Community On-Demand Virtual Care provides medical care where it's most convenient for you - at home, in the office or wherever you are.
Use on-demand virtual care to treat minor illness or injuries. Additional services include lab orders and prescription requests to your preferred pharmacy (non-narcotic medications only).
Virtual visits with our board-certified providers are done by phone or through a secure video on your computer or mobile device. The service is available from 7 a.m. to 11 p.m. daily.
Best of all, visits are FREE for plan members, spouses, dependents and retirees!
IMPORTANT: The service "Virtual Primary Care" is not available to Warren Township participants. If selected, you will be asked to pay for your visit.

Patricia English, MDDr. English graduated in 1989 from IUPUI with bachelor of science degree in chemistry. She graduated from Indiana University School of Medicine in 1993 and completed her family practice residency at St. Francis Hospital. Dr. English has been a family practitioner on the south side of Indianapolis since 1993. She joined Community Physician Network in December of 2011.
She is married to Jeffrey, a middle school math teacher, and they have four adult children. In her spare time, Dr. English enjoys reading, baking and playing with their dogs.
