Diabetes Care Clinics

Community Health Network Diabetes Care Clinics provide education and specialty care to patients struggling to gain control of their type 2 diabetes. The goal is to help patients get on the right path medically before returning them to their primary care doctor for ongoing management. Patients have access to a multidisciplinary team, including endocrinologists, diabetes educators, nurse practitioners, pharmacists and social workers.

Services

Services offered include:

  • Medication management
  • Glucose monitoring options
  • Nutrition and exercise education
  • Prevention of acute and chronic complications

If patients do not sustain their A1c goal, they can return to the Diabetes Care Clinic to manage and monitor closely until within goal again.

Referrals

Appropriate patients are non-pregnant adults with T2DM who have not been able to reach glucose goals (A1c still >8%) despite 6 months of medical management.

Patients with T1DM or gestational diabetes will still need to be seen at the endocrinology practice locations.

For ambulatory referrals in EPIC, search for specialty "Endocrinology" and choose "Diabetes Management Clinic".

Meet Our Team

Tara Myers, MD, endocrinologist and diabetes specialistTara Myers, MD
Medical Director/Provider

Dr. Tara Myers grew up in a small town in Central Indiana. She double majored at Indiana University where she received a bachelor of science in biology and bachelor of arts in psychology, after which she obtained her medical degree from Indiana University School of Medicine.

Her advanced medical training consists of an internship in medicine/pediatrics, internal medicine residency, followed by an endocrinology fellowship, all obtained through the Indiana University School of Medicine.

With her special training, Dr. Myers enjoys treating all areas in endocrinology.

Stephanie Tafflinger, NPStephanie Tafflinger, MSN, APRN, FNP-BC
Nurse Practitioner

Stephanie received a bachelor of science in nursing degree from Ball State University in 2011 and has been with Community Health Network for her entire nursing career. Stephanie worked as a medical/surgical RN at Community Hospital South for three years before transitioning to Labor and Delivery for the past eight years. She loves being a part of many families' most special days.

Her passion for nursing began after she was diagnosed with Type 1 Diabetes as an adolescent. Upon receiving her master of science degree in nursing from Northern Kentucky University in 2022, she knew she wanted to pursue a career where she could support others in managing diabetes.

Stephanie's hope is that patients will feel a connection with her since she has personal experience with the daily challenges of living with diabetes. Stephanie aims to provide patients with tips and strategies to help them manage diabetes in a positive way.  

In her personal time, Stephanie enjoys camping, watching sports, attending concerts and going on vacations with her family. She loves being able to cheer on her two young boys as they play sports.
