Diabetes Care Clinics

Community Health Network Diabetes Care Clinics provide education and specialty care to patients struggling to gain control of their type 2 diabetes. The goal is to help patients get on the right path medically before returning them to their primary care doctor for ongoing management. Patients have access to a multidisciplinary team, including endocrinologists, diabetes educators, nurse practitioners, pharmacists and social workers.

Looking for diabetes care? A referral is required to be seen at the Diabetes Care Clinic. Ask your primary care doctor if the clinic is a good option for you.

Services

Services offered include:

  • Medication management
  • Glucose monitoring options
  • Nutrition and exercise education
  • Prevention of acute and chronic complications

If patients do not sustain their A1c goal, they can return to the Diabetes Care Clinic to manage and monitor closely until within goal again.

Referrals

Appropriate patients are non-pregnant adults with T2DM who have not been able to reach glucose goals (A1c still >7%) despite 6 months of medical management.

Patients with T1DM or gestational diabetes will still need to be seen at the endocrinology practice locations.

For ambulatory referrals in EPIC, search for specialty "Endocrinology" and choose "Diabetes Management Clinic".

Meet Our Team

Daniel Brown, MD, diabetes care specialistDaniel Brown, MD

Daniel Brown, MD, completed his medical degree at Indiana University School of Medicine and an internal medicine residency at Ascension St. Vincent, with an obesity medicine fellowship through the Obesity Medicine Association. Before joining Community, Dr. Brown was a primary care provider in Hamilton County, treating a wide range of conditions and developing a strong focus on diabetes care.

Dr. Brown’s clinical interests center on diagnosing and treating adult type 2 diabetes and related conditions such as obesity, hypertension, dyslipidemia, metabolic-associated steatotic liver disease and chronic kidney disease. He also provides care for latent autoimmune diabetes in adults and adult type 1 diabetes.

In his free time, Dr. Brown enjoys watching movies and has collected nearly 1,500 titles. He lives in Indianapolis with his family and three dogs.
