House Calls and Care

Community ProCare at Home is a house-call care service for patients who are unable to leave their home for care, are recovering from a recent hospital stay, or may have worsened symptoms of a chronic illness. The team is happily seeing patients in Hamilton, Howard, Johnson, Madison and Marion County.

Services

You will receive care from a highly qualified team of care providers that includes a doctor and nurse practitioners (meet the clinical team below). The team can provide the following services in the comfort of your home:

  • Acute care visits, including follow-up visits
  • Annual wellness exams
  • Transitional care management visits upon discharge from a hospital or skilled nursing facility
  • Chronic disease management

During a visit, our providers can:

  • Provide most services usually offered in a doctor’s office
  • Help you identify risks and/or safety concerns in the home
  • Manage your blood work, imaging and other testing as needed at home
  • Connect you with the right referrals and community-based support
  • Provide you with more health education

Community ProCare at Home does not replace your primary care provider; the Community ProCare team serves as an add-on to your primary care practice and works closely with your doctor.

Meet Our Clinical Team

Jereana Miller, MSN, FNP-BC

Jereana Miller, family nurse practitionerJereana Miller is a board certified family nurse practitioner. A native of Chicago, she received her bachelor and master of science degrees in nursing from Indiana Wesleyan University. She is board certified by the American Nurses Credentialing Center as a Family Nurse Practitioner. She is a member of the ANA, AANP and CAPNI.

Jereana enjoys working with various populations, with an interest in chronic disease management and health promotion.

In her spare time, she enjoys volunteering for her local community through service work at local free clinics and provides care to underserved populations through mission work. Jereana is a mother of three and grandmother of two. She enjoys traveling, music, reading inspirational books, and spending time with family and friends.
