If you are interested in applying for a Community Health Network residency program, please use PhORCAS to submit all application materials. These will include a letter of intent, curriculum vitae, three references (please use ASHP form), and official college transcript(s).
PhORCAS
Application materials should be received no later than January 5, 2025 at 11:59 pm. Upon evaluation of application materials, qualified applicants will be offered an interview with the residency advisory team. Interviews take place in late January and February. Successful candidates will be expected to have a Doctor of Pharmacy degree, have an active BLS certification and obtain Indiana licensure within 90 days of the start of the residency year. PGY2 candidates are expected to have completed an ASHP-accredited PGY1 residency and obtain Indiana licensure within 90 days of the start of the residency year. The application process will include a formal employment application to be completed upon successful matching with our program.
*In 2025, the visit/interview process will be conducted in person.
