Charles (Chuck) Platz, MD is a family physician practicing with Community Family Medicine Care in Indianapolis. He serves as medical director for Controlled Substance Resources and chairs the Department of Family Medicine. He is a former chief of staff at Community Hospital East, Community Hospital North and Community Heart and Vascular Hospital, and a former chair of the Community Physician Network board.
Dr. Platz graduated from Grinnell College, and completed medical school at the University of Iowa College of Medicine. Following a surgical internship, he completed his training at Community Health Network’s Family Medicine Residency Program. He is Fellow of the American Academy of Family Physicians and has been a clinical faculty member at Community Health Network since 1991.
